too often it seems legitimate issues get lost when it seems the player is just whining about what happened. at this point, using /thor, jumping on crops and changing spawners is somehow legal (i dont like all of it, but it is what it is). using a format makes a complaint easier to sort out and deal with. it's also a good practice (and i've seen as a rule elsewhere) to not comment on a report you had nothing to do with unless you are the staff dealing with the issue. a clear format i've seen & used is:

    Your In-game name:
Offending Player's In-game name: (to find a real name if a nickname is used do /realname (nickname)
    Reason for ban request:
Proof of offence: ((i usually write "see screenshot" since nothing can be done without proof))
    Any witnesses In-game names:
    Description of damage caused:
Others who may be affected by offender:
Time of incident: (( Including timezone, makes it easier to check logs))
Area of server: ((usually global chat or /msg))

witnesses and others offended can also be a "see screenshot" answer if there's multiple people online - i will sometimes /list or /online, hit "t" to open chat and then screenshot that.

i've also seen too many people give up posting a report because they couldn't find their screenshots -

while there is a way to search for your minecraft screenhots folder, this is the easiest way i've found to get to it. from your options menu, choose Resource packs, then click "Open resource pack folder". go up one folder level to ".minecraft" then look for your "screenshots" folder. at this point, i recommend right-clicking and creating a shortcut, then drag the shortcut to your desktop or another easy to find folder.

also, you can hit "t" to bring up chat and fn f2 to screnshot while in chat, as well as using your scrolling to go back a bit further in chat if you missed something.

hope this helps to keep OSB a fun community that new players are excited to play in.
